For the safety and health of our patients and staff, we are implementing and enforcing the following protocols:
You must have an appointment to be seen. If you do not notify us in advance and miss your appointment, you will be assessed a $50 no-show fee that will be billed to you and not covered by insurance. This is to ensure that patients who are ill and need to be seen can be seen promptly. Unless the patient is under the age of 18 and needs a parent with them, only the patient will be taken back into the exam room. We ask that all family members or friends who come with you wait in the waiting area or outside the office. If you are experiencing respiratory symptoms, we ask that you call and press extension 3 to the nurse’s station and discuss your symptoms in detail so we can triage you appropriately. In 80% of cases, symptoms are mild, but for those who are over age 60 and/or immunocompromised, the mortality rate is 3.6% for those over the age of 60 up to 22% over the age of 80. For everyone’s safety, we want to remain cautious.
We ask that current and new patients contact us at (469) 645-0200 or email us at info@southernhorizon.org if you have any questions. We thank you for your patience and understanding during this time.
Office Policies
CANCELLATION / NO SHOW POLICY:
Southern Horizon Healthcare has implemented a Missed Appointment Policy effective January 1, 2016. There will be a $50 missed appointment fee for appointments that are missed without notifying our office 24 hours in advance. Your insurance company will NOT be billed for the missed appointment fee. You will be responsible for the fee of $50 regardless of your insurance plan. This is an office-wide policy that will assist us in becoming more efficient with our time and be able to offering missed appointment slots to other patients.
REFILL POLICY:
All controlled substances require a face-to-face office visit at each refill unless prearranged with your provider. Under no circumstances will a refill of medications of any kind occur after business hours. Routine prescriptions will require follow-up visits every three to six months, however timeframe may vary based on consultation.
AFTER OFFICE HOURS CARE:
An answering service is available to you after business hours for urgent questions and concerns. Call the office number, and it will automatically transfer you to the service at that time. Routine phone calls will not be transferred. Instead, a message will be taken and relayed to the office the next business day. You will receive a return call during the next business day.
